Instructions: The Business Assessment form can be filled out on-line then
saved as a Word Document to your computer. You can then e-mail it as an attachment
to sbdc@frii.com. Or, if you do not wish to complete the form on-line, simply print out
the form, hand-write the information and return to our office in order to schedule
your first counseling appointment. A Business Assessment, completed by the client PRIOR
to the first appointment will be the basis for the INITIAL counseling seesion.
For your advance information: At your first appointment, please be prepared to
show proof of lawful presence as identified on this Affidavit of Lawful Presence
form. This required form will be available at the SBDC for your signature.
